What to expect from your visit

Welcome to our practice.

The doctors and staff of the San Gabriel Valley Eye Associates are committed to quality medical and surgical eye care.In order to insure that your experience with us is professional and meets or exceeds your expectations, we would like to provide you with some information about our practice and what to expect when you arrive.

Parking
. There is ample parking directly in front of the medical office building. The fee for parking in this lot is $1.00 for the first two hours with a maximum fee of $4.00. We do not validate parking so please bring sufficient cash. Please do not park on the surface streets as there is a two hour limit and the local police department will ticket your car if you go over the limit.

Estimated Time of Your Visit.
If this is your first time in our office, please allow three hours from the time you are scheduled until the time you can expect to leave. We take pride in the thoroughness of our exams and our care in communicating with you. Neither of these should be rushed. Additionally, we must complete the necessary paperwork in order to properly bill your insurance.

If you are returning to our office for a follow-up exam please allow at least one hour for an undilated exam and at least two hours for a dilated exam. If you are scheduled for testing allow forty-five minutes for each test to be performed.

If you have another commitment scheduled on the same day as your visit with us, please leave sufficient time to safely drive from our office to your next scheduled activity. Also consider that dilation (which can last up to six hours) can impair your driving ability.

What to bring with you. (1) a valid photo ID; (2) your insurance card; (3) glasses or contact lenses if you wear them; (4) eye drops if you use them; (5) a list of your prescription medications.

Insurance. As a courtesy to you, we will bill your insurance if we are participating providers. Any balance that is not paid by Medicare or your insurance will be your responsibility. Applicable co-payments and deductibles will be collected at the time of the office visit, and are payable by cash, check, or credit card. Please read the enclosed Financial Policy for more information.

We strive to make your experience with us a positive one. We have included a survey for you to fill out after your visit with us is completed, in order to address any issues or comments you may have regarding your experience. You may drop this in the box at the reception counter or mail it back. Our receptionist would be happy to provide you with a stamped pre-addressed envelope for this purpose.

Sincerely,

Nancy Barone
